Educate Medicaid beneficiaries about, and enroll beneficiaries into, health plans. Saginaw CAC provides education to help the beneficiary choose a health plan and Primary Care Physician (PCP), informs them which doctors accept the health plans available to them and answers question regarding how to use the health plan. Saginaw CAC will also assist in changing the beneficiary's health plan (if within the first 3 months of selection of a plan).

Eligibility criteria are:

• You must have completed an application form at Department of Human Service (DHS)
• DHS determines Medicaid eligibility

To apply for service, bring

• Enrollment letter, if you have one
• Walk ins are welcome.
